Dr. Leverett's Phoenix plastic surgery practice is conveniently located in the beautiful Ahwatukee Foothills. We are fully equipped to perform many procedures in the comfort and privacy of our office, and also provide direct access to the Valley's best hospitals and surgery centers where Dr. Leverett is on staff.
Your Office Visit
We see all patients by appointment and evening appointments are available. However, in an emergency, we're always available. As part of our commitment to satisfaction, we have an education room with a variety of video presentations on cosmetic procedures, as well as brochures you can review. Financing is also available. Whatever your needs, you can confidentially discuss them with us in comfort and privacy.

Meet Our Team
Dr. Leverett understands the importance of assembling an outstanding medical team to help him uphold his mission of providing the highest standards of patient care. He is thrilled to work with competent medical professionals and support staff who strive to cater to each patient’s needs in a warm and comforting environment.
Shirley Kelly, RN, BSN
Shirley works alongside Dr. Leverett administering non-surgical facial rejuvenation treatments, including Botox, dermal fillers, IPL treatments and more. She has been in the field for more than seven years and is an instructor and clinical trainer for Allergan, the maker of Botox Cosmetic, and the International Association for Physicians in Aesthetic Medicine.
Shirley frequently appears on local television and radio programs, offering her expert knowledge and discussing the latest trends in non-surgical facial rejuvenation procedures.
Shirley obtained her nursing degree from Arizona State University, where she trained under the mentorship of accomplished dermatologists. Her fine-tuned aesthetic judgment and years of experience allow Shirley to provide our Phoenix plastic surgery patients with outstanding medical care for healthy and beautiful skin.

Dr. Leverett understands many patients feel apprehensive about undergoing plastic surgery.
